The Caring Mission's Geriatric Care Managers provide families with knowledge
and guidance to meet the needs of their loved ones. Our Care Managers
conduct a comprehensive assessment to include health, nutritional,
psychosocial, and financial concerns that an older adult may be
facing. All assessments are completed in the privacy of the client’s
home with or without family members present.
We arrange and coordinate services to eliminate the barriers identified
in the assessment process. The Caring Mission's Geriatric Care Managers
are dedicated to the client and provide ongoing services through
monitoring, communication with family (local or out of state), and
client's physician, when required. Our Care Managers are connected
with the community and have the resources to identify which programs
work, their reputations, strengths, and weaknesses.
